Initial Accreditation Application - Required Data Tables and Inventories

The tables listed below are pdf documents contained in a portfolio and work best with Adobe Reader 9.1 which is freely available at http://get.adobe.com/reader/.
 
When you open the portfolio, the tables will appear as document icons. To view them as a list, click "list view," the second icon from the left on the task bar.
 
For ease of navigation, we recommend that to view a table you open it in a window separate from this portfolio window. This can be done in two ways. The most direct is to right click on the document icon and select "open file." The desired document will open in a second window, leaving the portfolio visible in the first. Alternatively, double click on the document icon and the document will appear within the portfolio window. Then click the "open" button in the right top corner of the document. This will open the document in a separate window. This same document will also be visible in the original portfolio window. To return to the portfolio view, click the "home" or "list view" icons.
 
You can also move sequentially among the tables within the portfolio. To do so, from the portfolio view, double click on a document icon and the document will appear in the portfolio window. Use the arrows in the top left hand corner to move from one document to the next. Use the scroll bar to move within a document. To return to the portfolio view, click the "home" or "list view" icons.
